CCM Coordinator

We are seeking a CCM Coordinator to support Chronic Care Management programs and coordinate ongoing patient care activities.

The CCM Coordinator will help coordinate patient outreach, care management workflows, documentation, and communication between patients and clinical teams.

The role requires strong organization, communication, and an understanding of healthcare coordination processes.

• Coordinate patient enrollment and engagement activities.
• Support scheduled patient outreach and follow-ups.
• Maintain accurate patient records and documentation.
• Coordinate information between patients and clinical teams.
• Track care management activities and workflows.
• Assist with reporting and operational documentation.
• Follow established CCM processes and guidelines.
• Support patient engagement and communication initiatives.

• 2+ years of experience in healthcare operations or care coordination.
• Understanding of Chronic Care Management workflows.
• Strong communication and interpersonal skills.
• Excellent organizational abilities.

• Ability to work collaboratively with clinical teams.
